Never read, never seen.
Never heard of the book prior to the movie publicity tours, so would not have gotten it in March.
Would not be able to give the author's name
Added bit of info: I probably wouldn't have remembered the title if it hadn't been lifted from Shakespeare. Nicholas Sparks's generic phrases spring to mind as titles that wouldn't have sprung to mind. I would have figured out what they were referring to, then flailed to remember what it was called.
Ads for the book started appearing in the NYC subways - maybe last Christmas? Made me curious, but they didn't reveal much with that plain cover. I looked it up, but I still might not have gotten this clue until the movie came out. Since then I've read a lot about John Green, including a New Yorker profile that mentioned the dedication.
Hard to miss this on cultural radar, I would have thought.

Didn't do any pre-work, but I'm sure this would've been on the list if I had. I have seen the cover of the book, since I gave it to my 12 year old daughter. She has also seen the movie. I did not read it, but knew the very basics of the plot. Just didn't make the connection within 30 seconds.
I missed the clue, haven't read the book, have been wanting to see the movie but haven't yet, and saw the mention of it in the teen tournament precalls thread, but I still couldn't remember the title when I saw the FJ clue.
Got it. Haven't read it or seen it, but I will eventually. I can't say for sure that I would have gotten it in March, but the movie came up regularly when Shailene Woodley was doing interviews for Divergent, so I was at least tangentially aware of it.
John Green shows up on Mental Floss and YouTube and other places demonstrating lists of trivial facts. This blog post at mentalfloss.com is a good place to start.
Note: YouTube is blocked here at work, so if any of those links are dead, I do apologize.
I knew of the book and the author at least a year ago, and saw it in the pre-call thread.
I mentioned in the game thread that dh and I went to see the movie in NYC because we were exhausted after an overnight flight and really needed a nap. The Fault in Our Stars seemed like a good quiet choice. I woke up for about the last half hour, and will probably go see the movie again after I read the book.

Seems like an expensive nap. I prefer the hotel's bed for that purpose!
Ah, overnight flight. Too early to check into your hotel. Now I'm beginning to understand...
Green (starting at about 01:56): "I was really good friends with a young woman named Esther to whom the book is dedicated and Esther died of cancer in August of 2010 when she was 16."

This was the day before I taped Millionaire; we spent time in NYC before taking the train to Stamford.
